This is a required form for all Patriot Ordnance Factory, Inc. (POF-USA) Dealers and is all-inclusive to dealer pricing on all available products. This form typically takes 5-10 minutes to complete. Please have a copy of your FFL ready. You may also fill out as much as you can, and “Save & Continue” the form later by entering an email address on the confirmation page (or just bookmarking a special link).
